NEMSIS eVitals.VitalGroup (logical model, blood-pressure slice)

One eVitals.VitalGroup instance, the elements the blood-pressure StructureMap consumes (Architecture §4.3: every Observation from a group shares eVitals.01). NV attributes ride as <element>_nv (7701xxx) so the maps can route them to data-absent-reason — never fabricate, never skip.
